Primary parent contact information. This should be a parent who lives with the child and can be the first point of contact for all preschool communications.
Please let us know about a second parent. This parent may live with the child or at another address. This section is optional, however, if the second parent is considered an emergency contact or may be sharing in drop-off and pick-up, we must have complete information.
Please provide us with an alternative contact in the event that we are unable to reach a parent listed above.

Do you grant permission for Oak Hills Christian Preschool to feature your child and/or family in any promotional material or content, including, but not limited to, pictures, quotes, and school work on the Oak Hills Church website, Oak Hills Church social media accounts, or printed material? Names, ages, and other personal information will always be kept anonymous unless we make a specific request for a specific, one-time instance.
PLEASE NOTE: Your child is not guaranteed enrollment until we have received a completed registration form and the $175 registration fee. We also give priority to returning families who have submitted all registration forms and fees by January 1, for the following school year beginning in September. Final selection of classes will be done during the second week of January, with families being notified either way soon after. Both the registration form and registration fee must be received before we can consider your child a place in the preschool.
